Green Form

This online Green Form consists of two parts.

Part 1 Club Section.
The secretary completes this part and by using the "submit form" button, sends this part to the Demonstrator/Judge/ Teacher/Speaker. There will be an invitation on the email send to the Demonstrator/Judge/Teacher/Speaker to open up their part of the form.

Part 2 Demonstrator/J/T/S Section
The Demonstrator/J/T/S will receive the email containing the link to open their part of the Green Form.
The Demonstrator/J/T/S completes Part 2 and by using the "submit form" button sends this part to the secretary.

Both the secretary and the Demonstrator/Judge/Teacher/Speaker will receive details of both Part 1 and Part 2 of the form in an email.
