Green Form

This online Green Form consists of two parts.

Part 1 Club Section.
The secretary completes this part and by using the "submit form" button, sends this part to the Demonstrator/Judge/ Teacher/Speaker. There will be an invitation on the email send to the Demonstrator/Judge/Teacher/Speaker to open up their part of the form.

Part 2 Demonstrator/Judge/Teacher/Speaker Section
The Demonstrator/Judge/Teacher/Speaker will receive the email containing a link to open their part of the Green Form.

The Demonstrator/Judge/Teacher/Speaker, completes Part 2 and by using the "submit form" button sends this part to the secretary, Green Form is complete.

What information do you need to have to fill in the Green Form?

Club Secretary's Email address
and Phone no.
Demonstrator/Judge/
Speaker/Teacher's Name, Email adress and phone no.
Your Venue's Eircode
All details discussed regarding fee, flower cost etc.

Change of Club Officers

What is this for?
When a new Club Committee is selected, you will need to notify AOIFA so that we know who to talk to

What information do you need to have to fill in the Green Form?
Your club's AOIFA membership number.
New Chairperson, Secretary and Treasurer's Name, Email address and phone no.

Annual AOIFA Club Affiliation

What is this for?
To register your club affiliation to AOIFA
the form will also have payment information for you.

What information do you need to have to fill in the Green Form?
Treasurer's Name, Email address and phone no.
Number of members in your club.
Confirmation of Insurance Cover.

Annual AOIFA Individual Member Affiliation

What is this for?
All Demonstrators/Judges/Teachers/Speakers
have to be individual members of AOIFA.
Club members can also be an individual member and receive relevant AOIFA information.

What information do you need to have to fill in the Green Form?
Just your information, ie. your club name, your name, email address and phone no.
