Green Form v2

The new online Green Form v2 consists of three parts.
Any Green Forms already submitted will remain to be in the “old” 2-part format. Going forward, newly submitted Green Forms v2 will use the updated 3-part format
Part 1 Club section.
The club’s secretary fills in this part, information needed for this are:
1: The venue’s full address, including Eircode, if you don’t know the Eircode use
https://finder.eircode.ie to find it. 2: Email address and phone number of the secretary + an alternative contact phone number. 3: Event type, ie. club night, gala night, subject: ie. Demonstration, teaching dem, work night, judging competition or show, speaking or other. 4: Approx. number of attendees 5: Agreed fee and flower allowance 6: The club’s preferred method of payment and of course 7: The demonstrator’s name.
Upon submitting the form, the secretary will receive a submission email, which can be printed if you want/need a hardcopy and the demonstrator will receive an email invitation to open and fill out their Part 2 of the Green Form.

Part 2 Demonstrator section.
The demonstrator checks the information filled in on the form and can agree or disagree with some items. They will add their travel expenses, based on the travel distance calculated with the help of Google maps and Toll charges if required. Their approximate arrival time at the venue and if they need help unloading. If they require overnight accommodation and any other details.
Upon submitting the form, they will receive a submission email, which can be printed if a hardcopy is wanted/needed and the club’s secretary will receive an email invitation to open and complete Part 3.

Part 3 Booking Confirmation
The secretary checks the information and confirms the booking by selecting “yes, we agree”.
Upon submitting the form both the secretary and the demonstrator will receive the Booking Confirmation by email, which can be printed if a hardcopy is wanted/needed.
Green Form is complete.
To print an email: on top right you will see three little dots, click and select “print”.
(Based on Gmail being used, other email programs might use a slightly different method)
